The C100 SARS-CoV-2 Real-Time RT PCR test detects the presence of the underlying virus that causes COVID-19 and is for use with patients who meet current guidance for evaluation of infection. Many COVID-19 patients are asymptomatic, so testing those patients must also be considered.

Limit of Detection (LOD): 1000 (cp/mL)

Test Interpretations
• A positive result is considered definitive evidence of infection
• If no viral sequence is present, amplification will not occur, resulting in a negative result

A negative result means that no SARS-Cov-2 RNA was detected in the sample because it was either absent or below the limit of detection. Therefore, test results need to be considered in the context of patient symptoms and exposure history.

The C400 ImmunoBlot IgG test can determine if a patient’s immune system has produced IgG antibodies against the SARS-CoV-2 infection. IgG antibodies typically start developing 10 days from the onset of symptoms, and suggest that the patient may be in the recovery phase.

Sensitivity: Based on a study performed on 65 samples from 33 patients, between 0- 54 days after Real time RT-PCR was positive for SARS-CoV-2, 81.8% had SARS-CoV-2 specific IgG antibodies.

Specificity: 98.2% (n=221)

Test Interpretations for IgG Antibodies
• A sample is considered positive if two of the following bands are present: 75 kDa or 58 kDa, 50 kDa, and 25 kDa
• A sample is considered indeterminate if one of the following bands is present: 75 kDa, 58 kDa, and 25 kDa
• Negative: No SARS-CoV-2 specific IgG antibodies detected.

**Cross-reactivity to the other coronaviruses has not been tested.  This test is under FDA review.

The COV1 ImmunoBlot Panel 1 test detects IgM and IgG antibodies against the infection. The body makes IgM antibodies when first infected with a new virus, usually after seven days. IgG antibodies typically start developing 10 days from the onset of symptoms, and suggest that the patient may be in the recovery phase.

Sensitivity: Based on a study performed on 65 samples from 33 patients, between 0- 54 days after positive by Real time RT-PCR for SARS-CoV-2, 93.9% had antibodies to SARS-CoV-2.  72.7% had IgM antibodies and 81.8% had IgG antibodies.

Specificity: IgM: 99.1%; IgG: 98.2% (n=221)

The COV2 SARS-CoV-2 Complete Panel 2 includes C100 and COV1. This test panel is the recommended option because it provides the most comprehensive view of exposure to COVID-19 by testing for both active infection and antibody response. It helps determine if someone could potentially spread the virus, as well as determine if that person is immune from further infection.

IGENEX RECOMMENDS TESTING WITH A PANEL

IGeneX recommends testing for both active infection and antibody response due to overlapping periods of active infection, initial IgM antibody response, and antibody conversion from IgM to IgG. The chart below illustrates the overlap. Without testing for both, some patients will be missed.

1. The virus is detectable by RT-PCR at the onset of symptoms for up to 3 weeks (solid blue line)

2. The virus can be isolated up to 1 week after the onset of symptoms (solid red line).

WHY AN IMMUNOBLOT IS A SUPERIOR COVID-19 ANTIBODY TEST

In an ImmunoBlot, SARS-CoV-2 antigens are sprayed onto a nitrocellulose membrane. If specific antibodies to SARS-CoV-2 antigens are present, they will bind to the corresponding antigen bands. A dark purple precipitate will develop on the antigen-antibody complexes. Based on the studies to date, the ImmunoBlots have superior specificity and sensitivity compared to ELISA and Lateral Flow assays.

– ~93% sensitivity because multiple antigens are used
– Antigens are individually present on the blot

– Poor sensitivity, especially when antibody levels are low (early disease for IgM; late disease for IgG)

– Specific and pure recombinant antigens are used
– ELISA tests are screening tests and therefore not very specific, e.g. cross-reactivity to other viruses will give false positives for IgM and IgG

Collect & Ship Sample
A Sample is collected and sent to IGeneX as described in the test requisition form and in accordance with CDC specimen collection and shipping  guidelines.

Test is Performed
IGeneX conducts the test based on the product that was ordered. The test will be a PCR test conducted on a nasopharyngeal swab, or an ImmunoBlot test conducted using a blood sample.

Results are Provided
Results will be provided to the patient and physician within 24-48 hours for PCR testing, and within 3-5 business days for ImmunoBlots. Positive results will also be reported to the County and State Health Departments.
